About This Research

This report presents the first bilingual AI visibility audit of Dubai’s plastic surgery and aesthetic medicine market, conducted by Geometrika — BARS Agency’s proprietary Generative Engine Optimisation (GEO) measurement platform.

The study measured how five leading AI platforms — ChatGPT, Claude, Gemini, Grok and Perplexity — respond to 100 patient queries (50 in English, 50 in Russian), with Google AI Overviews and Yandex AI measured separately across the same query set. In total, the research generated and analysed 500 LLM responses, tracking brand mentions, citation sources and response positioning across 40 clinics and surgeons active in Dubai’s aesthetic medicine market. Data was captured on 16 July 2026.

What We Measured

  • 40 brands across two segments:
    Established multi-service clinics (including CosmeSurge, American Hospital Dubai, Lucia Clinic, Kings College Hospital, Cocoona, EDEN Aesthetics, Seline, Bizrahmed, Dubai Cosmetic Surgery and Euromed) and 18 individually tracked surgeons and personal-brand practitioners.
  • 100 queries across two language markets:
    English-language queries covering commercial intent (“best cosmetic surgeon Dubai”, “Botox Dubai best clinic”) and Russian-language queries reflecting how a large share of Dubai’s medical tourism patients actually search (“клиника пластической хирургии Дубай”, “лучшая клиника косметической хирургии”).
  • Metrics per brand:
    Mention Rate, Platform Consensus Score (0–5), Position Score (line of first mention), AIO Delta (the gap between Google AI Overviews and LLM average visibility), and citation domain count.

Key Findings

English AI and Russian AI are two different markets with two different leaders.
Seline is the #1 brand in Russian-language AI at 16.8% — and ranks 19th in English AI at 4.8%. The reverse holds for American Hospital Dubai, which ranks 2nd in English (17.6%) and only 8th in Russian (7.2%). A clinic measuring its AI presence in one language only is reading half the story, and often the less commercially relevant half.

The same brand, the same platform, a 28-point swing based on language alone.
Bizrahmed scores 4% in English LLM responses — effectively invisible. Ask Claude the same question in Russian, and Bizrahmed appears in 36% of relevant responses, its strongest result of any brand-platform combination in the study.

One clinic built near-perfect Russian AI visibility without a GEO strategy.
EDEN Aesthetics registers just 2% in English LLM responses, but edenderma.com is cited 107 times across Russian-language AI platforms — more than every aggregator in the market combined, and the single most-cited domain in Russian AI for this sector. This happened through the structure of the clinic’s existing web content, not a deliberate optimisation effort — which suggests what’s achievable once one exists.

No brand has a stable identity across platforms.
Seline’s Russian visibility ranges from 44% on Perplexity to 0% on Gemini — a 44-point spread for the market’s nominal #1 brand. Dr Rory McGoldrick swings from 38% on Grok to 0% on ChatGPT and Gemini. A brand strong on one platform can be entirely absent from another on the identical query.

Regulatory and aggregator domains outrank clinic websites.
dha.gov.ae is the most-cited source in English-language AI — ahead of any clinic’s own site. Bookimed, Whatclinic, Doctify and RealSelf are each cited more frequently than most individual clinic domains. For this market, third-party citation architecture currently carries more weight with AI platforms than owned content alone.

Personal brands are structurally more fragile than clinics.
Surgeons average 4.4% AI visibility in English and just 1.6% in Russian, against clinic averages several times higher. Nine of the eighteen tracked surgeons register zero visibility in at least one language. The mechanism is concrete: AI platforms cannot reliably disambiguate “Dr Kayle” from “Dr Dany Kayle” from “Dr D. Kayle” across sources, and that name fragmentation destroys citation count — regardless of the surgeon’s actual reputation or patient volume.

Google AI Overviews and conversational AI reward different signals.
Kings College Hospital shows the largest AIO Delta in the study: Google AI Overviews cites the brand in 44% of relevant queries, versus an 11.2% average across LLM platforms — a 32.8-point gap. Strong SEO and regulatory authority is not automatically translating into conversational AI presence.

Nine brands are invisible in English AI. Twelve are invisible in Russian.
These are not obscure operators — several have active clinics, professional websites and consistent social presence. Zero current AI visibility reflects a content and citation architecture gap, not a lack of reputation — and it is a closable gap.

Why does my clinic rank differently in English and Russian AI search?
AI platforms build their answers from different pools of indexed content for each language. A clinic with strong English-language SEO and citation architecture (Google, review platforms, English press) can be nearly invisible to Russian-language AI if its Russian content, directory listings and citation sources aren’t built out separately — and vice versa. The two are optimised independently, not automatically.

What is GEO (Generative Engine Optimisation)?
GEO is the practice of structuring a brand’s content, entity data and citation footprint so that AI platforms — ChatGPT, Claude, Gemini, Grok, Perplexity, Google AI Overviews and Yandex AI — surface and cite that brand when answering user queries. It sits alongside traditional SEO but optimises for how AI models select and cite sources, rather than how search engines rank pages.

Why do individual surgeons show up less than clinics in AI search?
AI platforms rely on consistent entity signals to recognise and cite a brand. A clinic typically has one consistent legal name, a licensed address and a stable web presence. A surgeon’s name is often spelled inconsistently across sources (“Dr Kayle” vs “Dr Dany Kayle” vs “Dr D. Kayle”), which fragments citation count and suppresses AI visibility — independent of the surgeon’s actual reputation or patient volume.

How was this AI visibility data collected?
Geometrika ran 100 identical patient queries (50 in English, 50 in Russian) against ChatGPT, Claude, Gemini, Grok and Perplexity, with Google AI Overviews and Yandex AI measured separately on the same query set — producing 500 tracked LLM responses across 40 Dubai-based clinics and surgeons on 16 July 2026.

Research Limitations

This study represents a single measurement snapshot taken on 16 July 2026. AI responses are non-deterministic and may vary across repeated runs. Mention Rate measures frequency of brand appearance, not sentiment, recommendation strength or clinical quality. A Position Score above 9 typically indicates the brand appears in narrative body text rather than a structured list, which measurably reduces real-world patient recall. Brand selection reflects market presence as of July 2026. Russian-language queries reflect standard transliterated search behaviour; regional dialect variation was not modelled. Scope excludes dentistry, orthodontics and dermatology-only clinics. No paid placement or sponsored content relationship exists between BARS Agency and any brand in this study.
